Klinikailag mem\u00f3riazavar, af\u00e1zia, funkci\u00f3veszt\u00e9s, demencia, a vizu\u00e1lis t\u00e9rbeli k\u00e9pess\u00e9gek k\u00e1rosod\u00e1sa \u00e9s v\u00e9grehajt\u00f3 funkci\u00f3zavar jellemzi. Jelenleg az Alzheimer-k\u00f3r olyan s\u00fcrget\u0151 t\u00e1rsadalmi \u00e9s orvosi k\u00e9rd\u00e9ss\u00e9 v\u00e1lt, amellyel glob\u00e1lisan foglalkozni kell. Ez\u00e9rt az \u00faj AD-gy\u00f3gyszerek fejleszt\u00e9s\u00e9nek helyzete tov\u00e1bbra is s\u00falyos.<\/p>\n<p>Jelenleg az Alzheimer-k\u00f3r jellegzetes k\u00f3ros elv\u00e1ltoz\u00e1sainak kutat\u00e1sa sor\u00e1n meg\u00e1llap\u00edtott\u00e1k, hogy az amiloid b\u00e9ta feh\u00e9rje (A \u03b2) k\u00f3rosan lerak\u00f3dik a beteg agy\u00e1nak felsz\u00edn\u00e9n, szenilis plakkokat k\u00e9pezve; a Tau feh\u00e9rje t\u00falzott foszforil\u00e1ci\u00f3ja pedig a beteg agy\u00e1ban neurofibrill\u00e1ris csom\u00f3k kialakul\u00e1s\u00e1hoz vezet az idegsejtekben, amit a gliasejtek burj\u00e1nz\u00e1sa k\u00eds\u00e9r. Jelenleg az Alzheimer-k\u00f3r patogenezise f\u0151k\u00e9nt a \"\u03b2 - amiloid feh\u00e9rje (A \u03b2) hipot\u00e9zis\", \"Tau feh\u00e9rje hipot\u00e9zis\", \"oxidat\u00edv stressz elm\u00e9let\", \"f\u00e9mion anyagcsere zavar elm\u00e9let\", \"neuroinflamm\u00e1ci\u00f3s hipot\u00e9zis\", \"kolinerg k\u00e1rosod\u00e1s elm\u00e9let\" stb. r\u00e9sz\u00e9t k\u00e9pezi. A kolinerg k\u00e1rosod\u00e1s elm\u00e9lete a legkor\u00e1bban javasolt elm\u00e9let az Alzheimer-k\u00f3r patogenezis\u00e9vel kapcsolatban, \u00e9s jelenleg ez az elm\u00e9leti alapja a legt\u00f6bb Alzheimer-k\u00f3r elleni gy\u00f3gyszer kifejleszt\u00e9s\u00e9nek is. Ez a hipot\u00e9zis azt sugallja, hogy a kolinerg aktivit\u00e1s cs\u00f6kken\u00e9se \u00f6sszef\u00fcgg az AD-betegek t\u00fcneteinek s\u00falyoss\u00e1g\u00e1val. Az AD-betegek boncol\u00e1sa sor\u00e1n az el\u0151agy baz\u00e1lis r\u00e9gi\u00f3j\u00e1ban l\u00e9v\u0151 neuronok elveszt\u00e9s\u00e9t, az acetilkolin-\u00e9szter\u00e1z \u00e9s a kolin-acetiltranszfer\u00e1z cs\u00f6kkent aktivit\u00e1s\u00e1t, ez\u00e1ltal a kolin felv\u00e9tel\u00e9re \u00e9s szint\u00e9zis\u00e9re val\u00f3 k\u00e9pess\u00e9g cs\u00f6kken\u00e9s\u00e9t, a tanul\u00e1si \u00e9s mem\u00f3riafunkci\u00f3k cs\u00f6kken\u00e9s\u00e9t, valamint kognit\u00edv k\u00e1rosod\u00e1st mutattak ki. Ez\u00e9rt az Alzheimer-k\u00f3r kezel\u00e9s\u00e9nek mindig is fontos ir\u00e1nya volt az olyan gy\u00f3gyszerek kifejleszt\u00e9se, amelyek k\u00e9pesek jav\u00edtani a szervezet kolinerg rendszer\u00e9nek m\u0171k\u00f6d\u00e9s\u00e9t.<\/p>\n<p>Az acetilkolin\u00e9szter\u00e1z (AChE) a szerin-hidrol\u00e1zok oszt\u00e1ly\u00e1ba tartozik, \u00e9s f\u0151k\u00e9nt az idegrendszerben van jelen. Akt\u00edv k\u00f6zpontja f\u0151leg h\u00e1rom r\u00e9szb\u0151l \u00e1ll: a hidrol\u00edzis hely\u00e9b\u0151l, anionokb\u00f3l \u00e9s hidrof\u00f3b r\u00e9gi\u00f3kb\u00f3l. Az AChE a biol\u00f3giai idegvezet\u00e9s kulcsfontoss\u00e1g\u00fa enzime, amely k\u00e9pes kataliz\u00e1lni a neurotranszmitter acetilkolin (ACh) hidrol\u00edzis\u00e9t a kolinerg szinaptikus hasad\u00e9kban kolinn\u00e1 \u00e9s ecetsavv\u00e1, megsz\u00fcntetni a jelingerl\u00e9st, \u00e9s blokkolni az idegjelek norm\u00e1lis tov\u00e1bb\u00edt\u00e1s\u00e1t a szervezetben. Az acetilkolin\u00e9szter\u00e1z-g\u00e1tl\u00f3 (AChEI) olyan anyag, amely reverzibilisen k\u00e9pes g\u00e1tolni az AChE-t. Felhalmozhatja az ACh-t a szinapszisokban, n\u00f6velheti annak tartalm\u00e1t, biztos\u00edthatja az idegi jelek norm\u00e1lis tov\u00e1bb\u00edt\u00e1s\u00e1t a szervezetben, \u00e9s jav\u00edthatja a tanul\u00e1si \u00e9s mem\u00f3riafunkci\u00f3kat. Jelenleg ez a klinikai gyakorlatban a legsz\u00e9lesebb k\u00f6rben alkalmazott AD-kezel\u0151 gy\u00f3gyszer.<\/p>\n<p>A term\u00e9szetes term\u00e9kek jelenleg a gy\u00f3gyszerek legnagyobb \u00e9s legjobb forr\u00e1s\u00e1t jelentik, olyan el\u0151nyeik miatt, mint p\u00e9ld\u00e1ul a szerkezeti sokf\u00e9les\u00e9g.
