Perkutan koronar intervention (PCI) er en meget vigtig minimalt invasiv behandlingsmetode for koronararteriesygdom (CAD). PCI kan effektivt forbedre den koronare blodgennemstr\u00f8mning, reducere angina og i h\u00f8j grad forbedre patienternes livskvalitet. Men den h\u00f8je forekomst af vaskul\u00e6r restenose (RS) efter angioplastik p\u00e5virker ofte den langsigtede behandlingseffektivitet. Tidligere unders\u00f8gelser har vist, at spredning og migration af vaskul\u00e6re glatte muskelceller (VSMC'er) er n\u00f8glefaktorer i udviklingen af vaskul\u00e6r restenose efter angioplastik. Efter endotelskade omdannes VSMC'er i intima fra kontraktile til syntetiske under p\u00e5virkning af forskellige v\u00e6kstfaktorer, cytokiner og inflammatoriske mediatorer, og de migrerer mod subendometriet. Overproliferation er et vigtigt trin i dannelsen af neointima. Derfor er effektiv h\u00e6mning af VSMC-proliferation og -migration blevet et centralt fokus i forebyggelsen og behandlingen af vaskul\u00e6r restenose. Danggui-polysaccharider er de vigtigste bioaktive komponenter i den traditionelle kinesiske medicin Danggui, som spiller en vigtig rolle i antiinflammatorisk, antitumor- og immunregulering. Der er dog endnu ikke rapporteret om anti-RS-effekten af Angelica-polysaccharider. I dette eksperiment blev Angelica sinensis-polysaccharider brugt til at behandle humane vaskul\u00e6re glatte muskelceller (hVSMC'er), man observerede VSMC'ernes spredning, migration og invasionsstatus og unders\u00f8gte den mulige molekyl\u00e6re mekanisme for Angelica sinensis-polysaccharider, der regulerer VSMC-funktionen, hvilket giver strategier for tidlig indgriben og behandling af vaskul\u00e6r restenose.<\/p>\n<p>Aktive stoffer fra traditionel kinesisk medicin har i lang tid v\u00e6ret brugt til at forebygge vaskul\u00e6r restenose efter PCI p\u00e5 grund af deres minimale bivirkninger. Forskning har vist, at Angelica-polysaccharider kan forbedre det h\u00e6matopoietiske mikromilj\u00f8 i knoglemarven ved at regulere de h\u00e6matopoietiske stamceller i knoglemarven. Desuden kan Angelica-polysaccharider forbedre akut leverskade hos rotter for\u00e5rsaget af for meget acetaminophen (APAP) ved at h\u00e6mme oxidativ stress og hepatocyt-apoptose. Mekanistisk set kan den biologiske aktivitet af Angelica sinensis-polysaccharider opn\u00e5s ved at regulere flere signalveje. Danggui-polysaccharider regulerer hypoxi-induceret apoptose og autofagi i neurale stamceller fra rotter ved at nedregulere BNIP3 og aktivere mTOR- og Notch-signalveje. Den seneste forskning har vist, at Angelica-polysaccharider h\u00e6mmer v\u00e6kst, migration og invasion af Hela-celler fra livmoderhalskr\u00e6ft ved at regulere p38-signalvejen. Virkningerne og de potentielle mekanismer af Angelica-polysaccharider p\u00e5 spredningen af vaskul\u00e6re glatte muskelceller og reparationen af beskadigede blodkar er dog ikke blevet unders\u00f8gt. Og yderligere demonstrere, at den h\u00e6mmende effekt af Angelica sinensis polysaccharidserum p\u00e5 hVSMC-spredning, migration og invasion opn\u00e5s ved at h\u00e6mme proteinaktivitet i PI3K\/AKT-signalvejen.<br \/>\nDet er velkendt, at Ang \u2161 er en kendt patogen faktor for aterosklerose, hypertension og vaskul\u00e6r restenose. If\u00f8lge rapporter er h\u00f8je niveauer af HB-EGF forbundet med en h\u00f8j risiko for stentrestenose. Resultaterne af denne unders\u00f8gelse viste, at Ang II og HB EGF i h\u00f8j grad kan inducere spredning, migration og invasion af hVSMC'er. Det er v\u00e6rd at bem\u00e6rke, at Angelica-polysaccharider kan reducere sprednings-, migrations- og invasionsniveauerne af hVSMC'er induceret af Ang II og HB-EGF betydeligt. Derudover er nogle migrationsregulerende proteiner, herunder celleadh\u00e6sionsmolekyler ICAM1, VCAM-1, MMP2 og MMP9, blevet rapporteret som vigtige reguleringsproteiner for unormal VSMC-migration. MMP'er, is\u00e6r MMP2 og MMP9, aktiveres hurtigt efter vaskul\u00e6r skade. Resultaterne af denne unders\u00f8gelse viser, at Angelica polysaccharidserum kan h\u00e6mme ekspressionen af MMP2 og MMP9 induceret af Ang II- og HB-EGF-stimulering.<br \/>\nPI3K\/AKT-signalvejen regulerer i vid udstr\u00e6kning cellul\u00e6r immunitet, proliferation, differentiering og apoptoseprocesser. Tidligere unders\u00f8gelser har rapporteret, at PI3K\/AKT-signalvejen spiller en vigtig rolle i aktivering af hVSMC-proliferation og -migration samt i neointimal hyperplasi efter vaskul\u00e6r skade. Aktiveringen af PI3K\/AKT-signalvejen kan f\u00f8re til restenose af blodkarrene inden i stenten. Tv\u00e6rtimod kan den nanopartikeleluerende stent, der indeholder AKT1 siRNA, effektivt lindre restenose i stenten. I den arterielle model for stentimplantation i hovedpuls\u00e5ren kan gefitinib-belagte balloner h\u00e6mme VSMC-proliferation og fremme celleapoptose ved at undertrykke PI3K\/AKT-signalvejen. Eksperimenter har ogs\u00e5 vist, at stimulering af humane lungearterie-VSMC'er med PDGF kan aktivere PI3K. PI3K regulerer ribosom S6-protein ved at fosforylere det nedstr\u00f8ms ribosomale protein p7OS6K af AKT og derved fremme VSMC-proliferation og migration. IGF-1 kan effektivt h\u00e6mme denne proces, hvilket viser, at PDGF inducerer VSMC-proliferation og migration p\u00e5 en PI3K-afh\u00e6ngig m\u00e5de. Zhou et al. simulerede den mekaniske stimulering af VSMC'er efter stent-implantation in vitro og fandt, at belastning af mus-VSMC'er med 5%- og 20%-potentiale i 6 timer kunne aktivere PI3K\/AKT-signalvejen og downstream GSK. Derudover h\u00e6mmer ginsenosid R1 VSMC-proliferation, migration og neointimal hyperplasi gennem PI3K\/Akt-signalvejen. Denne unders\u00f8gelse viste, at PI3K\/Akt-signalvejs-agonisten IGF-1 kan vende den h\u00e6mmende effekt af Angelica sinensis polysaccharid-serum p\u00e5 hVSMC-proliferation, migration og invasion.<br \/>\nSammenfattende kan Angelica polysaccharid-serum med succes h\u00e6mme spredning, migration og invasion af hVSMC'er stimuleret af Ang II og HB EGF.
