De spiller en central rolle i signalprocessen for mange cytokinreceptorer. Ved at aktivere downstream-m\u00e5lgener gennem signaltransducere og transkriptionsaktivatorer (STAT'er) ud\u00f8ver de deres regulerende effekt p\u00e5 cellul\u00e6re biologiske funktioner. JAK-STAT-signalvejen er involveret i vigtige biologiske processer som f.eks. celleproliferation, differentiering, apoptose og immunregulering. Unormal aktivering af JAK-STAT-signalvejen kan fremskynde spredningsprocessen af kr\u00e6ftceller, s\u00e5som akut promyelocytisk leuk\u00e6mi, hvis cellelinje HT93A inducerer differentiering og aktiverer JAK-STAT, hvilket f\u00f8rer til accelereret spredning. JAK-STAT-kanalen aktiveres ogs\u00e5 af NADPH-oxidase, som reducerer ekspressionsniveauet for lever X-receptor, i h\u00f8j grad p\u00e5virker ekspressionen af ATP-adenosintrifosfatbindingskassette (AB-CA1), h\u00e6mmer kolesteroludskillelsen af THP-1-skumceller og f\u00f8rer til patogenesen af \u00e5reforkalkning. P\u00e5 nuv\u00e6rende tidspunkt er forskning inden for medicin hovedsageligt fokuseret p\u00e5 kr\u00e6ft, inflammation og hjerte-kar-sygdomme, hvoraf inflammatoriske sygdomme hovedsageligt omfatter ulcer\u00f8s colitis, reumatoid arthritis og Crohns sygdom.<br \/>\nSom et vigtigt protein i menneskekroppen har JAK-proteinet mange sm\u00e5 molekyleh\u00e6mmere, der er designet til at ramme det. De fleste af dem befinder sig dog p\u00e5 forskellige stadier af den kliniske forskning p\u00e5 grund af lav oral biotilg\u00e6ngelighed og d\u00e5rlig selektivitet. Indtil videre er kun to JAK-h\u00e6mmere blevet godkendt til markedsf\u00f8ring af de regulerende myndigheder. Tofacitinib er en ny JAK1\/3-h\u00e6mmer, der er udviklet af Pfizer Inc. i USA. Den blev godkendt til markedsf\u00f8ring af den amerikanske FDA den 6. november 2021 og senere af den japanske PMDA den 25. marts 2013. Tofacitinib, som er en f\u00f8rstegenerations JAK-h\u00e6mmer, blokerer cytokinsignaltransduktion ved at h\u00e6mme downstream-signaler fra cytoplasmatiske receptorer og derved forstyrre JAK-STAT-signalvejen og p\u00e5virke DNA-transkriptionen. Det har spillet en vigtig rolle i antiproliferation og antiinflammatoriske effekter. Selvom l\u00e6gemidlet er blevet brugt i vid udstr\u00e6kning i klinisk praksis og har gode terapeutiske virkninger, har det ogs\u00e5 givet bivirkninger, herunder, men ikke begr\u00e6nset til, gastrointestinalt ubehag, kvalme, opkastning, an\u00e6mi, feber, \u00f8vre luftvejsinfektioner, forh\u00f8jede leverenzymer og blodlipider og i alvorlige tilf\u00e6lde \u00e5ndedr\u00e6tsbesv\u00e6r. Desuden \u00f8ger kombinationen af tofacitinib med potente immunosuppressiva som tacrolimus og cyclosporin ogs\u00e5 risikoen for immunsuppression, hvilket f\u00f8rer til en forv\u00e6rring af bivirkningerne.<\/p>\n<p>JAK3 er det eneste familiemedlem i JAK-familien, der er forbundet med cytokinreceptorens gamma-c-k\u00e6de. P\u00e5 grund af JAK3's ekstremt vigtige rolle i cytokinsignalering og dens ekspression kun i specifikke v\u00e6v, f\u00f8rer h\u00e6mning af JAK3-aktivitet til immunsuppression uden at for\u00e5rsage yderligere unormale fysiologiske \u00e6ndringer, hvilket g\u00f8r JAK3 til et vigtigt m\u00e5l for unders\u00f8gelse af immunsuppressiva. Derfor er der stadig et presserende behov for at udvikle JAK3-h\u00e6mmende l\u00e6gemidler med klare terapeutiske effekter og milde toksiske bivirkninger. Gennem virtuel screening med molekyl\u00e6r docking forventes sm\u00e5 molekyler af traditionelle kinesiske medicinkomponenter med lignende bindingssteder som tofacitinib at opn\u00e5 JAK3-specifikke h\u00e6mmere.<\/p>\n<p>Ovenst\u00e5ende dockingresultater viser, at bindingsevnen for sm\u00e5 molekyler glutarsyre, folsyre, secologanin, crotonosid, 2-O - (\u03b2 - D-glucopyranosyl) - ascorbinsyre (2-O - (\u03b2 - D-glucopyranosyl) - ascorbinsyre), tryptamin og vulganthin-I, som er kinesiske urteingredienser, til JAK3-kinase er st\u00e6rkere end for Tofacitinib, og de kan bruges som kandidat til JAK3-kinaseh\u00e6mmere. Det har v\u00e6rdi for yderligere forskning og design som en JAK3-kinasespecifik h\u00e6mmer. Malvaceae-glykosid findes i planter som kaprifolium og Malvaceae. Kaprifolie (Lonicera japonica Flos) er en traditionel kinesisk medicin i Kina, som har virkningerne af at forbedre immuniteten, antitumor, s\u00e6nke blodsukkeret og lipidniveauet, antiinflammatorisk og feberneds\u00e6ttende. Det har en lang historie med medicinsk brug og har en bred vifte af kliniske anvendelser; Strychnosnux vomica L. har vist sig at p\u00e5virke JAK-STAT-vejen, der er forbundet med udbruddet af reumatoid arthritis.
