Forskellige faktorer som oxidativ stress, inflammatorisk respons, fibroblastproliferation og -aktivering samt unormal ECM-aflejring er involveret i patogenesen af lungefibrose (PF). Endnu vigtigere er det, at processen med spredning og differentiering af lungefibroblaster til myofibroblaster (MFb) kan v\u00e6re n\u00f8glen til dannelsen af lungefibrose. Talrige unders\u00f8gelser har vist, at lungefibroblaster under stimulering af cytokiner s\u00e5som transformerende v\u00e6kstfaktor - \u03b2 1 (TGF - \u03b2 1) kan sprede sig og differentiere sig til alfa glat muskelaktin (\u03b1 - SMA) positiv MFb. MFb anses gennem sin syntese af en stor m\u00e6ngde ECM og pro-fibrotiske faktorer for at v\u00e6re en vigtig patogen mekanisme for udviklingen af lungefibrose. Derfor kan udforskning af mekanismerne for spredning og differentiering af lungefibroblaster og identifikation af deres h\u00e6mmende m\u00e5l v\u00e6re en potentiel strategi til at lindre lungefibrose.<\/p>\n<p>Isorhynchophylline (IRN) er et tetracyklisk indolalkaloid, der er isoleret fra den traditionelle kinesiske medicin Hook Vine. Tidligere unders\u00f8gelser har vist, at IRN har forskellige virkninger som antioxidant, antiinflammatorisk, antiproliferativ og neurobeskyttende. Vores tidligere forskning viste, at mimosa-alkaloider signifikant kan h\u00e6mme spredning og differentiering af lungefibroblaster induceret af TGF - \u03b2 1 ved at h\u00e6mme fosforyleringen af ekstracellul\u00e6re regulerede proteinkinaser 1\/2 (ERK1\/2) og opregulere ekspressionen af p27Kip1. IRN h\u00e6mmer signifikant proliferationen af neonatale rotteventrikul\u00e6re myocytter induceret af angiotensin II (AngII) ved at undertrykke phosphoryleringen af ERK1\/2. Baseret p\u00e5 ovenst\u00e5ende forskningsbaggrund har denne unders\u00f8gelse til form\u00e5l at unders\u00f8ge anti-PF-effekten og mekanismen for IRN ved at m\u00e5lrette ERK1\/2- og p27Kip1-signalvejene, hvilket giver nye ideer til forskning og udvikling af traditionel kinesisk medicin med anti-PF-effekter.<\/p>\n<p>Lungefibrose er en progressiv interstitiel sygdom, der er karakteriseret ved overdreven spredning af fibroblaster og aflejring af ekstracellul\u00e6r matrix, hvilket i sidste ende f\u00f8rer til \u00f8del\u00e6ggelse af lungestrukturen og dannelse af fibrotiske l\u00e6sioner med en d\u00e5rlig prognose. P\u00e5 nuv\u00e6rende tidspunkt er de vigtigste l\u00e6gemidler, der anvendes til behandling af lungefibrose, glukokortikoider og immunosuppressiva, men de har alvorlige bivirkninger. Traditionel kinesisk medicin og dens aktive ingredienser har vist god aktivitet mod lungefibrose ved at reducere udtrykket af inflammatoriske faktorer, h\u00e6mme spredningen af lungefibroblaster, fremkalde apoptose af lungefibroblaster og blokere fibroseprocessen. De kan effektivt forbedre fibrosesymptomer og forsinke sygdommens progression. Tidligere unders\u00f8gelser har vist, at monomerer af alkaloider i traditionel kinesisk medicin, s\u00e5som methylprednisolon og oxymatrin, har visse virkninger i forbindelse med lindring af lungefibrose. Vores forskningsgruppe fandt ogs\u00e5 i den tidlige fase, at mimosa-alkaloider og rutaecarpine har en vis lindrende effekt p\u00e5 bleomycininduceret lungefibrose. Andre unders\u00f8gelser har vist, at traditionelle kinesiske l\u00e6gemidler som Danshensu og Danshensu B forbedrer bleomycininduceret lungefibrose ved at h\u00e6mme spredningen og differentieringen af lungefibroblaster. Tidligere unders\u00f8gelser har vist, at IRN lindrer MCT-induceret PAH ved at h\u00e6mme spredningen af PASMC'er og reducere pulmonal vaskul\u00e6r remodeling og har en vis lindrende effekt p\u00e5 bronkial astma ved at h\u00e6mme spredningen af bronkiale glatte muskelceller. Denne unders\u00f8gelse viste imidlertid, at efter 21 dages behandling med IRN blev kollagenaflejring og ekspression af kollagen I i lungev\u00e6v fra PF-mus signifikant reduceret, ekspressionen af \u03b1 SMA blev signifikant reduceret, og graden af lungeskade blev signifikant reduceret. Yderligere in vitro-eksperimenter afsl\u00f8rede, at IRN kan h\u00e6mme TGF - \u03b2 1-induceret spredning af lungefibroblaster betydeligt, reducere TGF - \u03b2 1-induceret cellemigration betydeligt og i varierende grad h\u00e6mme udtrykket af TGF - \u03b2 1-induceret alfa-SMA og kollagen I i lungefibroblaster. Disse resultater tyder p\u00e5, at isorhynchophyllin kan forbedre BLM-induceret lungefibrose ved at h\u00e6mme spredning og differentiering af lungefibroblaster.<\/p>\n<p>Forskning har vist, at spredning, migration og differentiering af lungefibroblaster til MFb spiller en vigtig rolle i udviklingen af lungefibrose. Blandt dem spiller TGF - \u03b2 1 en n\u00f8glerolle i spredningen og differentieringen af fibroblaster til MFb. Forskning har fundet, at TGF - \u03b2 1 kan fremme spredning og differentiering af lungefibroblaster ved at aktivere Smad-signalvejen og ogs\u00e5 kan fremkalde spredning og differentiering af lungefibroblaster ved at aktivere MAPK-signalvejen. Cyclinafh\u00e6ngigt kinaseinhibitorprotein p27Kip1 er medlem af Cip\/Kip-familien af CDK-inhibitorer og en negativ regulator af proteinkinase CDK2\/cyclin E. Det kan blokere cellecyklussen i G0\/G1-fasen. Niveauet af p27Kip1 er relativt h\u00f8jt i G0\/G1-fasen af cellecyklussen. Under mitotisk stimulering nedbrydes p27Kip1 hurtigt, s\u00e5 CDK2\/cyclin E kan fremme celleproliferation. Vores forskningsgruppe fandt, at TGF - \u03b2 1 inducerer spredning og differentiering af lungefibroblaster ved at opregulere fosforyleringsniveauet for ERK1\/2 og h\u00e6mme ekspressionen af p27Kip1, mens ERK1\/2-inhibitoren PD98059 h\u00e6mmer spredning og aktivering af lungefibroblaster induceret af TGF - \u03b2 1 ved at h\u00e6mme ERK1\/2-fosforylering og opregulere ekspressionen af p27Kip1. I mellemtiden fandt vi ogs\u00e5, at mimosa-alkaloider h\u00e6mmede TGF - \u03b2 1-induceret proliferation og aktivering af lungefibroblaster ved at opregulere udtrykket af p27Kip1. Tidligere unders\u00f8gelser har vist, at IRN lindrer MCT-induceret PAH ved at h\u00e6mme fosforylering af ERK1\/2 og opregulere ekspressionen af p27Kip1 og derved undertrykke blodpladeaktiverende faktor-inducerede PASMC'er. Yderligere in vitro-eksperimenter afsl\u00f8rede, at IRN signifikant kan h\u00e6mme TGF - \u03b2 1-induceret ERK1\/2-phosphorylering, signifikant opregulere ekspressionen af p27Kip1 og signifikant nedregulere ekspressionen af CDK2\/cyclin E1.
