Nylige unders\u00f8gelser har vist, at PA's resistens \u00f8ges gennem quorum sensing under antibiotikabehandling. I de senere \u00e5r har PA p\u00e5 grund af den udbredte brug af antibiotika udviklet resistens over for flere antibiotika, hvilket g\u00f8r behandlingen af kliniske PA-infektioner betydeligt vanskeligere. Det er blevet et presserende globalt problem at finde nye l\u00e6gemidler til at undertrykke l\u00e6gemiddelresistente PA. I betragtning af de rigelige ressourcer af traditionel kinesisk medicin i vores region overvejer dette projekt at tage udgangspunkt i traditionel kinesisk medicin og m\u00e5lrette mod n\u00f8gleproteiner, der er involveret i quorum sensing. Ved hj\u00e6lp af molekyl\u00e6r docking s\u00f8ges der efter ligandmolekyler med h\u00f8j affinitet til aktive steder i den traditionelle kinesiske medicins forbindelsesbibliotek. Ved at bruge antibakterielle tests til at validere resultaterne af den virtuelle screening sigter vi mod at identificere kinesisk urtemedicin, der kan h\u00e6mme PA-resistens i klinisk praksis i kombination med antibiotika.<br \/>\nQuorum sensing (QS) er et intercellul\u00e6rt kommunikationssystem, der koordinerer udtrykket af virulens i bakteriepopulationer. Mekanismen best\u00e5r af fem punkter: (1) Overekspression af effluxpumpegener; (2) Inaktiverende enzymer hydrolyserer l\u00e6gemidler; (3) Biofilmdannelse; (4) M\u00e5lmutation; (5) Nedsat ydre membranpermeabilitet.<br \/>\nPA's quorum sensing-system omfatter mindst fire undersystemer: Pseudomonas kinolonsignal (PQS), integreret quorum sensing-signal (IQS) og LAS- og RHL-systemer. Sammenlignet med traditionelle antibiotika kan quorum sensing-inhibitorer (QSI'er) derfor reducere toksiciteten af PA, mindske l\u00e6gemiddelresistens, undg\u00e5 sepsis og forbedre f\u00f8lsomheden af antibiotikabehandling.<\/p>\n<p>Problemet med PA-resistens bliver stadig mere alvorligt, hvilket udg\u00f8r en betydelig udfordring for den terapeutiske effekt af antibiotika i klinisk praksis. QS-systemet er udbredt i forskellige bakterier, is\u00e6r PA, og udviser kaskaderegulering mellem systemerne, hvilket regulerer bakteriernes patogenicitet og l\u00e6gemiddelresistens. Sammenlignet med traditionelle antibiotika kan QSI'er h\u00e6mme QS-systemet hos m\u00e5lbakterier i passende koncentrationer uden at p\u00e5virke normal bakteriev\u00e6kst, reducere bakteriel virulens og h\u00e6mme spredningen af bakterietoksiner. De fem proteiner, der bruges i denne artikel, svarer til forskellige pathway-proteiner. Blandt dem er 5EOE et protein af typen BEL-1, der kan producere inaktiverende enzymer til at nedbryde \u03b2-lactamringen, hvilket f\u00f8rer til inaktivering af denne type antibiotika; 4HEF er et cephalosporin-hydrolasekompleks, der tilh\u00f8rer quorum sensing LAS-subsystemet. Det h\u00e6mmer serin \u03b2-laktase ved at danne et kovalent bindende enzymkompleks svarende til \u03b2-laktase, hvilket giver et strukturelt grundlag for dets bredspektrede h\u00e6mmende ydeevne; 4ZZL er en mutant af MexR-R21W, som er en h\u00e6mmer af MexAB OprM multidrug efflux pump operator i Pseudomonas aeruginosa. DNA-bindingsskade-mutationen f\u00f8rer til multiresistens. 4NR0 er en enoyl ACP-reduktase, der produceres til PA, og som spiller en vigtig rolle i syntesen af virulensfaktoren 3-oxo-C12HSL. 3H78 er et kompleks, der best\u00e5r af en signalbiosyntetisk enzymmutant af quinolon-pseudomonas og orthoaminobenzoat, som er en vigtig katalysator for biosyntese i bakterien. Inhibering af dette kompleks kan reducere produktionen af virulensfaktorer i PQS-systemet og mindske l\u00e6gemiddelresistens.<br \/>\nI de senere \u00e5r har unders\u00f8gelser vist, at nogle almindeligt anvendte kliniske l\u00e6gemidler ogs\u00e5 har QSI-aktivitet. Aspirin, som er et traditionelt feberneds\u00e6ttende og smertestillende l\u00e6gemiddel, har vist sig ikke at p\u00e5virke v\u00e6ksten af PA i en koncentration p\u00e5 6 mg\/ml, samtidig med at det har en vis h\u00e6mmende effekt p\u00e5 QS-virulensfaktorer som elastase og pyocyanin. Insektmidlet chloramphenicol kan h\u00e6mme produktionen af PA-virulensfaktorer og dannelsen af biofilm. Derudover har nogle kunstigt fremstillede l\u00e6gemidler ogs\u00e5 egenskaber som QSI'er. For eksempel kan et syntetisk derivat baseret p\u00e5 furanon (5Z) -4-brom-5-(brommethylen)-3-butyl-2-(5H)-furanon), furanon C-30, h\u00e6mme udtrykket af effluxpumper og virulensfaktorer, hvilket \u00f8ger PA-biofilmens f\u00f8lsomhed over for antibiotika. Men indtil videre er n\u00e6sten alle disse unders\u00f8gelser forblevet i laboratoriefeltet, og deres kliniske aktivitet har stadig brug for yderligere forskning.<br \/>\nDenne artikel bruger MOE-software til virtuelt at screene de aktive ingredienser i 2733 almindelige kinesiske plantel\u00e6gemidler og opn\u00e5r 7 kinesiske plantel\u00e6gemidler, der kan have h\u00e6mmende virkninger p\u00e5 l\u00e6gemiddelresistente stammer: galln\u00f8d, ramie-rod, f\u00e5rehovs\u00f8m, ildkulsmor, bladperle, vandfyr og flerblomstret vild p\u00e6on. Efterf\u00f8lgende blev der udf\u00f8rt relevant antibakteriel validering, og resultaterne var i overensstemmelse med de molekyl\u00e6re dockingberegninger, hvilket indikerer, at denne screening har en vis grad af rationalitet. Den virtuelle computerscreeningsmetode, der blev brugt i denne unders\u00f8gelse, er kendetegnet ved lave omkostninger og h\u00f8j effektivitet. At kombinere beregninger i traditionelle farmakologiske eksperimenter kan f\u00f8re til mere effektiv forskning. Det er dog v\u00e6rd at n\u00e6vne, at nogle traditionelle kinesiske l\u00e6gemidler ikke er blevet opn\u00e5et i virtuel databehandling, men har vist antibakteriel aktivitet i faktisk screening (s\u00e5som Yanfumu, Xiqingguo, Hezi osv.), hvilket indikerer, at denne metode stadig er relativt r\u00e5 med afvigelser og udeladelser og skal forbedres. I den senere fase vil molekyl\u00e6r dynamiksimulering blive brugt til at analysere vigtige videnskabelige sp\u00f8rgsm\u00e5l som systemets mobilitet og h\u00e6mningsmekanisme.<br \/>\nMed hensyn til de udvalgte kinesiske urter i denne artikel observerede Chen et al., at galln\u00f8dder under visse omst\u00e6ndigheder har en vis rensende effekt p\u00e5 PA-biofilm og endda fjerner den fuldst\u00e6ndigt; Song et al. fandt, at ekstraktet af den kinesiske l\u00e6geurt Charcoal har en vis h\u00e6mmende effekt p\u00e5 v\u00e6ksten af PA, og dens mindste h\u00e6mmende koncentration er 15,63 mg\/ml; Pandey et al. fandt gennem in vitro antibakterielle eksperimenter, at ekstrakter af f\u00e5rehovbille (blade, st\u00e6ngelbark og blomster) har en vis antibakteriel effekt p\u00e5 PA.<br \/>\nEt andet form\u00e5l med denne artikel er at give kliniske svar p\u00e5 antibiotikaresistente PA-infektioner. Vi samarbejder i \u00f8jeblikket med kliniske afdelinger om at udv\u00e6lge forskellige traditionelle kinesiske l\u00e6gemidler, som vil blive afkogt og kombineret med amoxicillin, cefotaxim og levofloxacin for at observere deres terapeutiske virkninger og g\u00f8re nogle fremskridt. Ud\u00f8vere af klinisk kinesisk medicin har dog givet feedback om, at der er brug for st\u00e6rkere traditionelle kinesiske medicinformler for at opn\u00e5 en bedre prognose. Selv om de fleste stoffer stadig befinder sig p\u00e5 stadiet for forskning i biologisk aktivitet eller pr\u00e6klinisk forskning, og der er relativt f\u00e5 stoffer, der g\u00e5r ind i den kliniske fors\u00f8gsfase, vil der stadig v\u00e6re en masse usikkerhed i fremtiden. QSI'er har gode anvendelsesmuligheder, og de rapporterede QSI'er er stadig begr\u00e6nsede.
