Traditionel kinesisk medicin og patentmedicinske pr\u00e6parater, der indeholder AA, anvendes i vid udstr\u00e6kning i klinisk praksis, herunder Aristolochia, Fangji, Vermillion, Asarum og Shanci Mushroom of Asarum samt traditionel kinesisk patentmedicin og enkle pr\u00e6parater som Compound Weitong Capsules, Chuanxiling Capsules, Shisanwei Shugan Capsules. P\u00e5 nuv\u00e6rende tidspunkt har Kina forbudt brugen af l\u00e6geurter med h\u00f8jt AA-indhold som Guangfangji og Guanmutong, mens Aristolochiaceae-planter med relativt lavt indhold og deres pr\u00e6parater stadig bruges i klinisk behandling. AAI er hovedkomponenten i AA, og indholdet af AAI varierer mellem forskellige kinesiske medicinske materialer. For eksempel er AAI-indholdet i Asarum omkring 10 \u03bc g\/g, mens AAI-indholdet i cinnober lotus varierer fra 0,9386 til 3,5675 mg\/g. N\u00e5r cinnoberlotus administreres klinisk med 5-10 g, n\u00e5r AAI-m\u00e6ngden op p\u00e5 mellem 9,386-35,675 mg. Denne artikel henviser til andres forskning. Efter at v\u00e6re blevet administreret med AAI20 mg \/ kg i 5 dage viste mus betydelig nyreskade: blodkreatinin og urinstofnitrogen steg begge markant; Glomerul\u00e6rt \u00f8dem, glomerul\u00e6r konsolidering og l\u00f8srivelse af proksimale tubul\u00e6re epitelceller.<br \/>\nRNA seq er en teknologi baseret p\u00e5 high-throughput-sekventering til at udf\u00f8re high-throughput-sekventeringsanalyse p\u00e5 alt mRNA i v\u00e6vsceller. Denne unders\u00f8gelse brugte transkriptomik til at analysere RNA fra AAI-behandlet musenyrev\u00e6v og fandt i alt 4975 forskelligt udtrykte gener sammenlignet med den normale gruppe. Blandt dem blev 2511 gener opreguleret, og 2464 gener blev nedreguleret. De top 5 forskelligt udtrykte gener var kap Spp1\u3001Aldh1a2\u3001Serpine1\u3001Tnc\u3002<br \/>\nKap er et renalt androgenregulerende protein, som er meget specifikt og strengt reguleret i renale proksimale tubul\u00e6re celler. Unders\u00f8gelser har vist, at samspillet mellem Kap og cyclophilin B kan beskytte proksimale tubul\u00e6re celler mod toksiciteten af cyclosporin A. Spp1, ogs\u00e5 kendt som osteopontin, er et ekstracellul\u00e6rt matrixkemokin som phosphoglykoprotein, der er forbundet med immunregulering, tumorigenese og cellesignalering. Unders\u00f8gelser har vist, at opregulering af Spp1 kan aktivere NF-\u03ba B-signalvejen for at fremme kr\u00e6ftprogression. Aldh1a2 tilh\u00f8rer aldehyddehydrogenase 1-familien og er involveret i syntesen af endogen all-trans-retinsyre (ATRA) under nyrernes udvikling. Som en aktiv metabolit af A-vitamin spiller ATRA en vigtig rolle i celleproliferation, differentiering og apoptose. Serpine 1 er en type 1 plasminogenaktivator-inhibitor. Mange unders\u00f8gelser har p\u00e5peget, at Serpine 1 har unormal ekspression i kr\u00e6ft, s\u00e5som oral pladecellekarcinom, esophageal karcinom, bl\u00e6rekr\u00e6ftkarcinom osv. Tnc er et ekstracellul\u00e6rt matrix (ECM)-glykoprotein med et stort molekyle, som er st\u00e6rkt udtrykt under embryonaludviklingen. Det kan binde sig til strukturelle ECM-proteiner og celleoverfladereceptorer (s\u00e5som EGFR, integrin), aktivere downstream-veje og regulere celleadh\u00e6sion, diffusion, migration og proliferation. Unders\u00f8gelser har vist, at Tnc er den vigtigste komponent, der fremmer fibroblastproliferation og spiller en n\u00f8glerolle i nyrefibrose. Men under akut nyreskade kan Tnc ogs\u00e5 induceres og derved \u00f8ge Wnt\/\u03b2 - catenin-signalering for at beskytte nyrerne. Der er kun f\u00e5 rapporter om sammenh\u00e6ngen mellem ovenst\u00e5ende gener og nefrotoksicitet ved AAI.<br \/>\nDe gener, der udtrykkes forskelligt i AAI-nefrotoksicitet, er hovedsageligt signifikant beriget i GO-klassifikationer s\u00e5som metabolisme af sm\u00e5 molekyler, ekstracellul\u00e6rt matrixv\u00e6v og immunresponser, der involverer neutrofiler. KEGG-analyse viste, at forskelligt udtrykte gener er involveret i JAK-STAT og NF-\u03ba B-signalveje; Peroxidationsreaktion; Glykolyse\/glukoneogenese; Insulinresistens; Signifikant berigelse er observeret i nedbrydningsvejene for valin, leucin og isoleucin.
