Den begynder normalt med almindelige symptomer som diarr\u00e9, mavesmerter, rektal bl\u00f8dning, feber, v\u00e6gttab og tr\u00e6thed. Ved CD str\u00e6kker bet\u00e6ndelsen sig fra slimhinden til serosa gennem tarmv\u00e6ggen, og denne sygdom forts\u00e6tter med at komme tilbage og aftage. Med flere tilbagefald kan CD udvikle sig fra mild til moderat inflammation til alvorlige penetrerende (fisteldannelse) eller stenotiske sygdomme, og mere alvorlige tilf\u00e6lde kan f\u00f8re til kr\u00e6ft, hvilket p\u00e5virker patientens livskvalitet alvorligt. Derudover udviser CD-patienter ogs\u00e5 mange ekstraintestinale inflammationer, s\u00e5som inflammatoriske reaktioner i \u00f8jne, lever, hud og led, hvilket afspejler denne sygdoms systemiske karakter. Behandlingen af CD omfatter f\u00f8rstelinjebehandling og andre behandlinger, hvoraf f\u00f8rstelinjebehandlingen omfatter steroider og anti-tumornekrosefaktor alfa (TNF - \u03b1), som hurtigt kan lindre symptomerne. Derfor er der et presserende behov for at udvikle alternative l\u00e6gemidler fra naturlige ressourcer og funktionelle f\u00f8devarer til behandling af CD.<\/p>\n<p>Nostoc commune er en bl\u00e5gr\u00f8n algeorganisme, der ofte bruges til forebyggelse og behandling af sygdomme p\u00e5 grund af sin omfattende og rige ern\u00e6ring. I de senere \u00e5r har ekstraktet vist sig at v\u00e6re effektivt til behandling af tarmsygdomme. I vores indledende forskning fandt vi ud af, at den vigtigste aktive ingrediens i t\u00f8ndersvamp, p-hydroxybenzaldehyd (HD), kan forbedre symptomerne p\u00e5 colitis ulcerosa (UC) ved at h\u00e6mme tarmens inflammatoriske respons. Baseret p\u00e5 de mange ligheder mellem UC og CD spekulerer vi i, at HD kan have anti-CD-effekter. For at verificere denne hypotese inducerede vi en CD-model med mus og en TNF-\u03b1-induceret Caco-2-celleinflammationsmodel ved hj\u00e6lp af 2,4,6-trinitrobenzensulfonsyre (TNBS) for at unders\u00f8ge den beskyttende effekt og mekanisme af HD p\u00e5 CD, hvilket giver videnskabeligt bevis for HD-rige f\u00f8devarer som en supplerende alternativ terapi til forebyggelse og behandling af IBD.<\/p>\n<p>&nbsp;<\/p>\n<p>CD er en idiopatisk kronisk inflammatorisk sygdom med ukendt \u00e6tiologi, som er tilb\u00f8jelig til at komme igen og p\u00e5virker hele ford\u00f8jelseskanalen fra munden til anus. Forekomsten er h\u00f8jest i Europa og Nordamerika, men forekomsten i den asiatiske befolkning er steget kraftigt i de senere \u00e5r. Forskning har vist, at patogenesen af CD er t\u00e6t forbundet med tarmbet\u00e6ndelse for\u00e5rsaget af dysregulering af tarmslimhindens immunrespons. Flere unders\u00f8gelser har vist, at forbedring af tarmens inflammatoriske respons kan lindre CD betydeligt. For eksempel forbedrer thalidomid CD hos mus ved at genoprette ubalancen mellem TH17\/Treg-celler og \u00e6ndre balancen mellem pro-inflammatoriske og anti-inflammatoriske cytokiner for at lindre inflammation. Maki-polyphenolekstrakt h\u00e6mmer tarmbet\u00e6ndelse og forbedrer CD hos mus ved at nedregulere udtrykket af COX-2 og iNOS. Ovenst\u00e5ende indikerer, at h\u00e6mning af tarmens inflammatoriske respons er et vigtigt middel til behandling af CD. I overensstemmelse hermed fandt vi, at HD kan forbedre TNBS-induceret CD ved at h\u00e6mme den inflammatoriske respons.<\/p>\n<p>Inflammation er hoved\u00e5rsagen til IBD og f\u00f8rer til skader p\u00e5 tarmv\u00e6vet. Som vigtige inflammatoriske biomark\u00f8rer omfatter relaterede cytokiner TNF - \u03b1, IL-1 \u03b2, IL-6 og IL-17, som spiller vigtige roller i det inflammatoriske respons. H\u00e6mning af deres overekspression kan forbedre den inflammatoriske reaktion i tarmen ved CD. For eksempel kan LB h\u00e6mme overekspressionen af IL-6, IL-1 \u03b2 og TNF-\u03b1 i tarmen hos mus, der er induceret af TNBS, og forbedre Crohns sygdom. Natriumbutyrat h\u00e6mmer inflammation og opretholder epitelbarrierens integritet i en musemodel med inflammatorisk tarmsygdom fremkaldt af TNBS. Ovenst\u00e5ende unders\u00f8gelser tyder alle p\u00e5, at h\u00e6mning af produktionen af proinflammatoriske cytokiner har en terapeutisk effekt p\u00e5 CD. I denne unders\u00f8gelse fremkaldte TNBS alvorlig tarmbet\u00e6ndelse hos mus. HD-behandling reducerede imidlertid infiltrationen af colitisceller og nedregulerede signifikant niveauerne af inflammatoriske cytokiner p\u00e5 b\u00e5de dyre- og celleniveau, hvilket indikerer, at HD kan h\u00e6mme det inflammatoriske respons og forbedre CD hos mus.<\/p>\n<p>NF - \u03ba B er en vigtig signalvej for transkriptionen af inflammatoriske cytokiner i IBD, der er ansvarlig for at regulere transkriptionen af proinflammatoriske cytokiner (TNF - \u03b1 og IL-6). Som medlem af NF-\u03ba B-familien kan p65, n\u00e5r den er aktiveret, translokeres til kernen for at opregulere det transkriptionelle udtryk af proinflammatoriske cytokiner. Forskning har vist, at h\u00e6mning af NF- \u03ba B-signalvejen kan lindre inflammation i TNBS-modellen. I denne unders\u00f8gelse \u00f8gede induktion af TNBS udtrykket af fosforyleret protein NF- \u03ba B p65 i tyktarmen hos CD-mus, mens intervention med HD f\u00f8rte til et fald i udtrykket af fosforyleret protein NF- \u03ba B p65. Derudover kan HD ogs\u00e5 h\u00e6mme opreguleringen af NF - \u03ba B p65-phosphoryleringsproteinekspression induceret af TNF - \u03b1 i Caco-2-celler. HD forbedrer tyktarmens inflammatoriske respons ved at h\u00e6mme NF-\u03ba B-signalvejen.<\/p>\n<p>AMPK er en serin\/threoninkinase, der spiller en vigtig rolle i glukose- og lipidmetabolismen, cellev\u00e6kst og apoptose, autofagi og inflammation. Forskning har vist, at AMPK opn\u00e5r antiinflammatoriske virkninger ved at h\u00e6mme aktiveringen af NF- \u03ba B. Xu et al. fandt, at aktivering af AMPK kan h\u00e6mme NF- \u03ba B-vejen og derved undertrykke inflammatoriske reaktioner under hypoxi og reoxygeneringsprocesser. Hu et al. fandt, at brugen af forbindelse C (AMPK-h\u00e6mmer) kan forhindre salidrosid i at h\u00e6mme NF - \u03ba B-signalvejen og forbedre AGE'er induceret endotelbet\u00e6ndelse og oxidativ stress. P\u00e5 samme m\u00e5de viste denne unders\u00f8gelse, at HD har en dosisafh\u00e6ngig effekt p\u00e5 fremme af AMPK-fosforylering. Ovenst\u00e5ende indikerer, at HD forbedrer TNBS-induceret inflammatorisk respons ved at aktivere AMPK for at h\u00e6mme NF-\u03ba B-signalvejen.<\/p>\n<p>PPAR \u03b3 er en transkriptionsfaktor, der spiller en vigtig rolle i antiinflammatoriske, antioxidante og fagocytiske cellemedierede clearanceprocesser og er st\u00e6rkt udtrykt i colonepitelet. Mange unders\u00f8gelser har vist, at PPAR \u03b3 er en aktivator af AMPK, og at den antiinflammatoriske effekt af PPAR \u03b3 opn\u00e5s ved at aktivere AMPK. For eksempel kan PPAR \u03b3-agonister (thiazolidinedion-l\u00e6gemidler) aktivere AMPK gennem fosforylering af AMPK \u03b1 for at h\u00e6mme tarmpatologisk inflammation. GW9662, T0070907 og PPAR \u03b3 siRNA kan forhindre aktivering af AMPK med hydroxyasiatisk syre for at genoprette Th17\/Treg-balancen og forbedre colitis.
