De fleste basalcellekarcinomer kan helbredes ved operation, men ved fremskredent eller progressivt basalcellekarcinom kan operation og\/eller str\u00e5lebehandling v\u00e6re utilstr\u00e6kkelig og kan endda for\u00e5rsage betydeligt tab af funktion eller udseende. Selvom platinbaserede kemoterapimidler har en betydelig antitumoraktivitet, er der ingen v\u00e6sentlig \u00e6ndring i patienternes overlevelsesrate. Derfor er det n\u00f8dvendigt at finde en ny behandlingsmetode, der kan forbedre prognosen for basalcellekarcinom. Basalcellekarcinom stammer fra follikul\u00e6rt epidermis eller basale keratinocytter i h\u00e5rs\u00e6kkene. I 95% af sporadiske basalcellekarcinomer er Hedgehog (Hh)-signalvejen unormalt aktiveret. En stor m\u00e6ngde forskningsdata har vist, at Hh-signalvejsh\u00e6mmere er yderst effektive behandlingsmuligheder for refrakt\u00e6re og inoperable basalcellekarcinompatienter, men det er ogs\u00e5 almindeligt at stoppe behandlingen p\u00e5 grund af u\u00f8nskede l\u00e6gemiddelbegivenheder. Derfor vil udviklingen af en ny, sikker og effektiv h\u00e6mmer af Hh-signalvejen forbedre de nuv\u00e6rende og fremtidige terapeutiske udsigter for basalcellekarcinom betydeligt. Icaritin er den vigtigste aktive monomere komponent i Epimedium, en plante, der tilh\u00f8rer Epimedium-sl\u00e6gten i Berberidaceae-familien. I de senere \u00e5r er antitumoreffekten af icariin blevet grundigt unders\u00f8gt, hvilket kan h\u00e6mme forskellige maligne tumoraktiviteter s\u00e5som livmoderhalskr\u00e6ft, spiser\u00f8rskr\u00e6ft, melanom osv. gennem forskellige signalveje og cellem\u00e5l. Blandt dem spiller Hh-signalvejen en vigtig rolle i icariins h\u00e6mning af proliferation, migration og invasion af spiser\u00f8rskr\u00e6ftstamceller. Denne unders\u00f8gelse vil fokusere p\u00e5 at udforske icariins antibasalcellekarcinom-aktivitet og dens forhold til Hh-signalvejen for at tilvejebringe nye potentielle l\u00e6gemidler til behandling af basalcellekarcinom.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>Hh-signalvejen er involveret i forskellige former for biologisk adf\u00e6rd, s\u00e5som initiering, migration og apoptose af tumorv\u00e6v, og er forbundet med dannelsen og udviklingen af op til en tredjedel af de ondartede tumorer. Hedgehog-proteinet kodes af mindst tre gener hos hvirveldyr: Sonic Hedgehog (Shh), Indian Hedgehog (Ihh) og Desert Hedgehog (Dhh). Shh udtrykkes bredt i huden, nervesystemet og ford\u00f8jelseskanalen og virker p\u00e5 to cellemembranreceptorer, Patched (Ptc) og Smothened (Smo). Under normale fysiologiske forhold er Hh-ligander ofte frav\u00e6rende, og Ptc binder sig til Smo, hvilket h\u00e6mmer Smo-proteinaktiviteten og dermed forhindrer nedstr\u00f8ms glioma-associerede onkogen (Gli)-transkriptionsfaktorer (Gli1, GliI2 og Gli3) i at blive overf\u00f8rt til kernen. N\u00e5r Hh-proteinet binder sig til Ptc, frig\u00f8res h\u00e6mningen af Smo, s\u00e5 Gli-proteinet i fuld l\u00e6ngde kan komme ind i kernen og aktivere transkriptionen af downstream-m\u00e5lgenet. Forskning har vist, at op til 95% af basalcellekarcinomer udviser unormal aktivering af Hh-signalvejen, hvor ca. 67% viser Ptc-mangel og 10% viser Smo-mutationer, hvilket f\u00f8rer til, at Gli kommer ind i kernen og fremmer celledeling og tumordannelse. Derfor kan m\u00e5lrettet h\u00e6mning af Hh-signalvejen effektivt undertrykke udviklingen af basalcellekarcinom uden v\u00e6sentlige virkninger p\u00e5 normale v\u00e6vsceller.<br \/>\nEpimedium bruges ofte som et n\u00e6rende og afrodisiakum i traditionel asiatisk medicin med en bred vifte af terapeutiske evner. Talrige in vivo- og in vitro-unders\u00f8gelser har vist, at icariin har en bred vifte af antitumoraktiviteter, der virker gennem forskellige mekanismer som apoptose, cellecyklusregulering, antiangiogenese, antimetastase og immunregulering. Blandt dem er h\u00e6mning af Hh-signalvejen den vigtigste mekanisme, hvormed icariin h\u00e6mmer spiser\u00f8rskr\u00e6ftceller. Denne unders\u00f8gelse viste, at icariin har en bred h\u00e6mmende effekt p\u00e5 Hh-signalvejsrelaterede faktorer (Hedgehog, Smo, Gli1) i basalcellekarcinom A431-celler, samtidig med at det medierer celleapoptose og h\u00e6mmer dens migration. Dette tyder p\u00e5, at icariin kan ud\u00f8ve anti-basalcellekarcinomeffekter ved at h\u00e6mme Hh-signalvejen.<br \/>\nGli er et kernemedlem af Hh-signalvejen, og dens downstream-m\u00e5lgener omfatter vejmedlemmerne Ptch1, Ptch2, Gli1 samt tumorcelle-apoptoseregulator Bcl-2, cellecyklusregulator CCND2 og cellemetastaserelaterede faktorer. MMP-9 er i \u00f8jeblikket en af de mest unders\u00f8gte matrixmetalloproteinaser, som kan spalte forskellige ekstracellul\u00e6re matrixproteiner, regulere ekstracellul\u00e6r matrixombygning og er t\u00e6t forbundet med tumorinvasion, metastase og angiogenese. Den er st\u00e6rkt udtrykt i tumorer som leverkr\u00e6ft, oral pladecellekarcinom og neuroblastom og er t\u00e6t forbundet med Hh-signalvejen. For yderligere at belyse forholdet mellem den h\u00e6mmende effekt af icariin p\u00e5 A431-celler og Hh-signalvejen brugte denne unders\u00f8gelse GANT 61 til at h\u00e6mme Gli og opn\u00e5 effekten af at blokere Hh-signalvejen. Resultaterne viste, at udtrykket af Gli1, Bcl-2 og MMP-9 blev signifikant nedreguleret, mens Bax blev signifikant opreguleret, og der blev observeret celleapoptose og migrationsh\u00e6mning. Epimedium har den samme effekt p\u00e5 reguleringen af ovenn\u00e6vnte cytokiner og adf\u00e6rd, men n\u00e5r Hh-signalvejen h\u00e6mmes, mister icariin sin regulerende effekt p\u00e5 Bcl-2 og Bax, forbedrer kun lidt sin h\u00e6mmende effekt p\u00e5 MMP-9, og celleapoptose og migration viste ikke signifikante \u00e6ndringer. Dette indikerer, at icariin ikke virkede signifikant p\u00e5 A431-celler gennem andre signalveje, men dens regulering af MMP-9 kan have andre signalveje, som stadig skal unders\u00f8ges n\u00e6rmere. Forskning.
