Arbejdet med de immunrelaterede mekanismer i denne sygdom er imidlertid utilstr\u00e6kkeligt, hvilket ikke bidrager til at forbedre patienternes langsigtede overlevelsesprognose. Derfor er denne unders\u00f8gelse den f\u00f8rste til yderligere at udforske de relevante immuninfiltrationsmekanismer og potentiel effektiv traditionel kinesisk medicin baseret p\u00e5 differentierede gener med det form\u00e5l at forbedre mekanismerne og fremme l\u00e6gemiddeludviklingen. For det f\u00f8rste screenede denne unders\u00f8gelse egnede genchips gennem GEO-databasen og sammenlignede genekspressionsprofilerne for bugspytkirteltumor og normale v\u00e6vspr\u00f8ver fra den samme individuelle patient og opn\u00e5ede 2653 forskelligt udtrykte gener. De centrale differentielt udtrykte gener blev identificeret gennem PPI, herunder ekstracellul\u00e6rt matrixv\u00e6v og kollagenfiberv\u00e6vsrelaterede gener (COL1A2, COL1A1, COL3A1, COL5A1, COL5A2), ADAM metallopeptidase (ADAMTS2), k\u00e6ldermembranpolysaccharid (LUM), integrin subunit alpha 2 (ITGA2), fibronectin (FN1) og matrix metalloproteinase (MMP2). Ovenst\u00e5ende forskelligt udtrykte gener har vist sig at v\u00e6re opreguleret i tumorv\u00e6v, relateret til kollagenmetabolisme, og tilh\u00f8rer for det meste COL1-, COL3- og COL5-familierne. Blandt dem blev udtrykket af COL1A2 bekr\u00e6ftet at v\u00e6re det h\u00f8jeste i v\u00e6vet ved qRT PCR, hvilket indikerer h\u00f8j p\u00e5lidelighed af dette resultat. I mods\u00e6tning til andre solide tumorer er PDAC-tumormikromilj\u00f8et et t\u00e6t matrixmilj\u00f8, der indeholder pankreatiske stellatceller, kr\u00e6ftrelaterede fibroblaster, immunceller og overdreven fibrose i den ekstracellul\u00e6re matrix. Hvordan man bruger dette punkt til at \u00e6ndre de kliniske resultater, er blevet et af de mulige gennembrud.<br \/>\nYderligere GO- og KEGG-berigelsesanalyser af forskelligt udtrykte gener tyder p\u00e5, at deres biologiske processer involverer neutrofil degranulering, migration, kemokiner, T-hj\u00e6lpercelledifferentiering, humoral immunrespons og andre processer. PDAC-mikromilj\u00f8et er hovedsageligt inflammatorisk, og det er ogs\u00e5 bevist, at kemokiner og andre faktorer fremmer det inflammatoriske mikromilj\u00f8 i tumorer. Ud over cytokiner og kemokiner er aktivering af neutrofiler ofte den f\u00f8rste molekyl\u00e6re begivenhed, efterfulgt af migration mod tumorv\u00e6v. Det er tydeligt, at PDAC-cancerceller kan rekruttere polymorfonukle\u00e6re neutrofiler i n\u00e6rheden af tumoren, men de fleste af dem kan ikke producere anti-tumorresponser. Typisk fremmer h\u00f8j overflod af neutrofiler progressionen af PDAC og er forbundet med d\u00e5rlig prognose; Nylige in vitro-eksperimenter har vist, at kr\u00e6ftceller i bugspytkirtlen inducerer dannelsen af ekstracellul\u00e6re f\u00e6lder af neutrofiler og derved fremmer forekomsten af levermikrometastase af PDAC; Samtidig bestemmer den lokale hypoxi for\u00e5rsaget af h\u00f8j fibrose immunmilj\u00f8et, og den kontinuerlige frigivelse af reaktive iltarter f\u00f8rer til lokal hypoxi og forbedrer dermed neutrofil infiltration. Derfor er neutrofiler afg\u00f8rende i mikromilj\u00f8et for kr\u00e6ft i bugspytkirtlen. Desuden tyder resultaterne af denne unders\u00f8gelse p\u00e5, at forskelligt udtrykte gener er direkte involveret i T-celledifferentiering og humoral immunregulering.<\/p>\n<p>For yderligere at udforske immuninfiltrationens rolle i patogenesen af PDAC brugte denne unders\u00f8gelse CIBERSORT-dekonvolutionsmetoden til at analysere de samlede tumorpr\u00f8ver. Sammenlignet med normalt bugspytkirtelv\u00e6v var M2-makrofager og CD4+T-celler i hvilehukommelse signifikant for\u00f8get i tumorv\u00e6v, mens CD8+T-celler var signifikant reduceret. Den samme tendens blev observeret i parrede pr\u00f8ver. Derfor er det meget sandsynligt, at makrofager bliver potentielle m\u00e5l for nye terapeutiske strategier for PDAC hos mennesker. Det blev tidligere antaget, at PDAC's immunsuppressive mikromilj\u00f8 best\u00e5ende af fibroblaster og fibroblaster ville begr\u00e6nse infiltrationen af T-celler, men nyere forskning har afsl\u00f8ret den specifikke rumlige fordeling af T-celler, da korrelationsvarmekortet viser, at der er en signifikant negativ sammenh\u00e6ng mellem CD4+T-celler i hvilehukommelsen og CD8+T-celler i bugspytkirtelkr\u00e6ftv\u00e6v (r=-0,64), hvilket har vist sig at v\u00e6re en af indikatorerne for d\u00e5rlig prognose, is\u00e6r PDAC. Resultaterne af case-serielitteraturforskning og metaanalyse tyder p\u00e5, at kombineret traditionel kinesisk medicinbehandling af PDAC effektivt kan reducere patientd\u00f8deligheden og forbedre den langsigtede prognose. Nyere grundforskning har afsl\u00f8ret, at mekanismen for traditionel kinesisk medicin til behandling af PDAC hovedsageligt er gennem forbedring af tumorens mikromilj\u00f8, hvilket indirekte forbedrer de medicinske resultater. Derfor er det afg\u00f8rende at udforske tumorens immunmikromilj\u00f8 for at finde effektiv traditionel kinesisk medicin. P\u00e5 nuv\u00e6rende tidspunkt antages patogenesen af PDAC i traditionel kinesisk medicin for det meste at v\u00e6re \"mangel p\u00e5 oprindelse, men overskud af essens\" med fokus p\u00e5 ophobning af fugtig varme og toksin. Blandt de kinesiske medicink\u00f8er med h\u00f8j korrelation blev det konstateret, at ginsenosider kan reducere angiogenese og tumorcelleern\u00e6ringsforsyning ved at nedregulere udtrykket af MMP2 og andre faktorer; Curcumin h\u00e6mmer selvfornyelse af PDAC-stamceller gennem aktiviteten af Wnt \/ \u03b2 - Catenin-signalvejen og kan forbedre lokal mikrocirkulation i PDAC subkutan transplantation af tumormodelv\u00e6v, \u00f8ge blod- og iltforsyningen i tumormikromilj\u00f8et og h\u00e6mme den hypoxiske stressrespons af tumorceller og stromaceller; Ovenst\u00e5ende litteratur underst\u00f8tter nogle af de forudsagte resultater af denne unders\u00f8gelse, men der mangler stadig forskning i immunmekanismerne relateret til traditionel kinesisk medicinbehandling af PDAC.<br \/>\nDenne unders\u00f8gelse tog h\u00f8jde for individuelle forskelle i analysen af immunmekanismer ved screening af genchips og screenede PDAC-eksperimentelle data grundigt ved hj\u00e6lp af analyse af parrede pr\u00f8ver.
