Fedtsyrer under 6 kulstof er d\u00e5rligt fordelt i naturen, og de metaboliske veje for laurinsyrer med 12 kulstof og langk\u00e6dede fedtsyrer er ens, s\u00e5 de typiske mellemk\u00e6dede fedtsyrer omfatter kun fedtsyrer med 8-10 kulstof.<\/p>\n<p>Mellemk\u00e6dede fedtsyrer, langk\u00e6dede fedtsyrer og glycerol udg\u00f8r triglycerider, dvs. mellemk\u00e6dede triglycerider (MCT) og langk\u00e6dede triglycerider (LCT), og ford\u00f8jelsen, absorptionen og metabolismen af disse to fedtsyrer er meget forskellig.<br \/>\n<strong>Figur 1 Ford\u00f8jelse, absorption og metabolisme af LCT og MCT.<\/strong><\/p>\n<p><strong>LCT<\/strong><\/p>\n<p><strong>1. Mundhulen<\/strong><br \/>\nN\u00e5r LCT kommer ind i munden, vil spytlipase for\u00e5rsage hydrolyse af triglycerider, og fedtsyrerne i triglyceridernes 3. position vil blive hydrolyseret for at producere 1,2-glyceroldiestere og frie fedtsyrer. Tyndtarm<\/strong><br \/>\nHydrolyse af triglycerider finder hovedsageligt sted i tolvfingertarmen. Under p\u00e5virkning af lipase fra bugspytkirtlen hydrolyseres et triglyceridmolekyle til to molekyler fri fedtsyre og et molekyle 2-glycerolmonoester. Dern\u00e6st danner frie fedtsyrer og monoglycerider (og fosfolipider, kolesterol osv.) en blandet mikroklynge med galdesyrer i ford\u00f8jelsessaftene, og den blandede mikroklynge absorberes af jejunum i den \u00f8verste del af tyndtarmen ved hj\u00e6lp af tyndtarmens villi; galdesyrerne i den blandede mikroklynge absorberes ikke med det samme, men kun lipidfraktionen. Efter absorptionen i tyndtarmen omdannes frie fedtsyrer og monoglycerider i tyndtarmens epitelceller til triglycerider, som derefter danner c\u00f8liakipartikler sammen med fosfolipider og kolesterolestere i lymferne.<\/p>\n<p><strong>4. Lymfe og blod<\/strong><br \/>\nC\u00f8liakipartikler passerer gennem lymfekarrene og smelter sammen med blod i vena subclavia og kommer dermed ind i blodcirkulationen. Nogle af triglyceriderne i blodet hydrolyseres til 2-glycerolmonoestere og frie fedtsyrer under p\u00e5virkning af lipoproteinlipase (LPL) og lagres i v\u00e6v og muskler i hele kroppen, mens de ikke-hydrolyserede triglycerider kommer ind i leveren.<\/p>\n<p><strong>5. Lever<\/strong><br \/>\nLeveren udf\u00f8rer fedtmobilisering, n\u00e5r kroppen har brug for energi. Triglycerider hydrolyseres og \u03b2-oxideres og nedbrydes fuldst\u00e6ndigt til kuldioxid, vand og energi.<\/p>\n<p><strong>Tabel 1 Sammenligning af LCT- og MCT-metabolisme<\/strong><\/p>\n<p><strong>MCT<\/strong><\/p>\n<p><strong>1. Mundtlig<\/strong><br \/>\nSpytlipase i mundhulen hydrolyserer de tre fedtsyrer i MCT meget hurtigere end LCT, s\u00e5 MCT bliver delvist hydrolyseret i mundhulen.<\/p>\n<p><strong>2. Maven<\/strong><br \/>\nDet meste af MCT hydrolyseres til glycerol og frie fedtsyrer i maven, og fordi MCT er hydrofilt, er det lettere at hydrolysere.<\/p>\n<p><strong>3. Tyndtarm<\/strong><br \/>\nDet meste af MCT kommer ind i tolvfingertarmen som frie fedtsyrer, s\u00e5 der er ikke brug for lipase i bugspytkirtlen. Desuden er MCT meget hydrofilt og kr\u00e6ver ikke deltagelse af galdesyrer for at danne blandede mikroklynger. Ligesom glukose og aminosyrer, som er nedbrydningsprodukter af sukker og proteiner, kan MCT transporteres direkte til leveren gennem portvenen. For nylig har det vist sig, at MCT, som ikke hydrolyseres i maven, kan absorberes direkte i tyndtarmen i form af triglycerider. Da der ikke er behov for lipase i bugspytkirtlen og galdesyrer, er MCT velegnet til patienter med ford\u00f8jelsesproblemer.<\/p>\n<p><strong>4. Lever<\/strong><br \/>\nN\u00e5r MCT n\u00e5r leveren gennem portvenen, gennemg\u00e5r det en hurtig \u03b2-oxidation og bliver til sidst nedbrudt til energi, kuldioxid og vand.<\/p>\n<p>\u03b2-oxidationsprocessen for MCT og LCT er ogs\u00e5 forskellig. Fedtsyreoxidation finder sted i mitokondrier, og LCT kr\u00e6ver hj\u00e6lp fra lipoylcoenzym A-syntetase, carnitin og carnitinlipoyltransferase (CPT-1, CPT-2, CPT-1 er det hastighedsbegr\u00e6nsende enzym for \u03b2-oxidation) for at komme ind i mitokondriernes ydre og indre membraner, mens MCT danner den mellemk\u00e6dede fedtsyre, lipoylcoenzym A, som kan passere direkte gennem mitokondriemembranen og underg\u00e5 \u03b2-oxidation som reaktion p\u00e5 lipoylcoenzym A's virkning. Derfor har MCT en hurtigere \u03b2-oxidationshastighed end LCT og genererer energi meget hurtigere.<\/p>\n<p>N\u00e5r MCT er blevet til mellemk\u00e6det fedtsyre lipoyl coenzym A, syntetiseres en del af det af leveren til triglycerider, fosfolipider og cholesterylestere osv, men denne andel er meget mindre end LCT. Det meste af det \u03b2-oxideres i mitokondrierne til acetylcoenzym A, som derefter indg\u00e5r i tricarboxylsyrecyklussen, ketogenesen, fedtsyresyntesen eller kolesterolsyntesen.
