Langvarig tilbagevendende kronisk inflammatorisk infiltration er en almindelig \u00e5rsag til kolorektal cancer, som i sidste ende kan udvikle sig til colitisassocieret kolorektal cancer (CAC) gennem gentagen stimulering af tarmv\u00e6vet. Derfor er udforskning af videnskabeligt effektive behandlingsstrategier til at kontrollere inflammatorisk infiltration et vigtigt middel til at forebygge og behandle kolorektal cancer. Selvom kemoterapi, str\u00e5lebehandling og kirurgisk behandling p\u00e5 nuv\u00e6rende tidspunkt har forbedret patienternes overlevelsescyklus betydeligt, er bivirkningerne ved kemoterapi og str\u00e5lebehandling blevet kritiseret af mange patienter. Derfor haster det med at finde meget effektive antiinflammatoriske l\u00e6gemidler med lav toksicitet til kolorektal cancer.<\/p>\n<p>Under langvarig klinisk l\u00e6gemiddeloverv\u00e5gning og behandling fandt vores afdeling, at den traditionelle kinesiske medicin Huangqin i form af en formel ofte bruges i den kliniske behandling af inflammatorisk kolorektal cancer og til at lindre udviklingen af inflammatorisk cancer. Det kan forbedre patienternes symptomer p\u00e5 tarmbet\u00e6ndelse betydeligt. Gennem litteraturforskning fandt man ud af, at Huangqin-afkog med Huangqin som hovedingrediens kan lindre dextransulfatnatrium (DSS)-induceret colitis hos mus ved at regulere antallet af Th17\/Treg-celler og h\u00e6mme frigivelsen af pro-inflammatoriske cytokiner. Baicalin er en flavonoidforbindelse, der stammer fra den traditionelle kinesiske l\u00e6geplante Scutellaria baicalensis Georgi. Moderne farmakologisk forskning har vist, at baicalin har forskellige farmakologiske aktiviteter som f.eks. antiinflammatorisk, antioxidant, antileverfibrose, myokardiebeskyttelse og antitumoreffekter. Baicalin-kapsler, med baicalin som den vigtigste aktive ingrediens, er blevet anvendt klinisk som adjuverende behandling af akut og kronisk hepatitis samt vedvarende hepatitis. I de senere \u00e5r er fordelene ved denne naturlige aktive ingrediens i behandlingen af kolorektal cancer gradvist blevet anerkendt, is\u00e6r som et adjuverende l\u00e6gemiddel til klinisk behandling af kolorektal cancer. Derudover har unders\u00f8gelser vist, at Hedgehog-signalvejen spiller en vigtig rolle i den inflammatoriske transformationsproces af kolorektal cancer, og dens unormale aktivering kan fremme flere processer s\u00e5som spredning, migration og invasion af kolorektale cancerceller. N\u00e5r Hedgehog-signalvejen aktiveres, \u00f8ges ekspressionen af Sonic Hedgehog (SHH), hvilket igen lindrer h\u00e6mningen af G-proteinkoblet receptor (SMO) aktivitet og derved lindrer aktiveringsprocessen af serin \/ threoninproteinkinase (SUFU). Baicalin er en planteafledt flavonoidforbindelse med forskellige funktioner s\u00e5som antiinflammatorisk, antimikrobiel infektion og anti-malign tumor. Baicalin-kapsler, der hovedsageligt best\u00e5r af baicalin, bruges i \u00f8jeblikket som adjuverende behandling af akut, kronisk og vedvarende hepatitis i klinisk praksis. I denne unders\u00f8gelse fandt vi, at baicalin kan h\u00e6mme aktiveringen af Hedgehog-signalvejen og derved undertrykke spredningen af SW620 kolorektale kr\u00e6ftceller, inducere celleapoptose og h\u00e6mme infiltrationen af inflammatoriske faktorer. Dette tyder p\u00e5, at baicalin kan v\u00e6re en lovende adjuverende behandling af antiinflammatorisk kolorektal cancer. Den h\u00e6mmende effekt af baicalin p\u00e5 spredningen af NCM460-celler i denne unders\u00f8gelse tyder dog p\u00e5 forsigtighed og undg\u00e5else af de h\u00e6mmende virkninger for\u00e5rsaget af h\u00f8je koncentrationer og langvarig eksponering af baicalin p\u00e5 NCM460-celler.<\/p>\n<p>Hedgehog-signalvejen er en vigtig signalvej, der er blevet opdaget i de senere \u00e5r, og som er involveret i forekomsten og udviklingen af kolorektal cancer. Dette er af stor betydning for, at baicalin kan kontrollere eller forsinke den ondartede f\u00e6notype af kolorektal cancer. Den regulerende effekt af dette kontrolpunkt p\u00e5 downstream kr\u00e6ftrelateret faktorbiologi skal dog stadig udforskes yderligere.<br \/>\nSom en etableret risikofaktor for kolorektal cancer kan langvarig tilbagevendende kronisk inflammation indirekte fremkalde genetisk ustabilitet og epigenetiske modifikationer gennem inflammatoriske faktorer, ROS og kv\u00e6lstofstoffer, hvilket f\u00f8rer til kr\u00e6ftudvikling. N\u00e5r kroppen st\u00f8der p\u00e5 indre\/ydre skader eller andre relaterede inflammatoriske stimuli, vil den aktivere immunsystemrelaterede inflammatoriske celler, som derefter udskiller en stor m\u00e6ngde cytokiner som IL-1 \u03b2, IL-6 og TNF - \u03b1 for at danne et nyt inflammatorisk infiltrationsmikromilj\u00f8 med den ekstracellul\u00e6re matrix, hvilket i sidste ende f\u00f8rer til kr\u00e6ft. If\u00f8lge litteraturrapporter p\u00e5virker effekten af at h\u00e6mme Hedgehog-signalering i tyktarmen ikke direkte selve de kolorektale epitelceller, men virker snarere p\u00e5 stromaceller for at inducere udtrykket af immunregulerende cytokin IL-10 og derved h\u00e6mme inflammatorisk tarmskade. Derfor skal det iboende forhold mellem de inflammatoriske faktorer, der er unders\u00f8gt i denne artikel, og Hedgehog-signalvejen stadig udforskes yderligere.<br \/>\nGenerelt er det vanskeligt at opn\u00e5 tilstr\u00e6kkelige anticancereffekter ved at regulere et enkelt sygdomsmolekyl\u00e6rt m\u00e5l under behandlingen af ondartede tumorer. Derfor anvender man i klinisk praksis ofte en kombination og synergistisk tilgang til behandling af kolorektal cancer for at forbedre effekten af kemoterapimedicin eller reducere toksiske bivirkninger. Den klassiske Hedgehog-kaskadesignalvej har vist sig at v\u00e6re t\u00e6t forbundet med sygdomsprogression og l\u00e6gemiddelresistens ved kolorektal cancer. P\u00e5 baggrund af dette er denne unders\u00f8gelse baseret p\u00e5 kliniske behov og belyser den molekyl\u00e6re mekanisme for baicalin, en effektiv komponent i traditionelle kinesiske medicinmonomerer, i bek\u00e6mpelsen af kolorektal cancer gennem de klassiske veje, der er n\u00e6vnt ovenfor. Dette vil give et sikkert grundlag for klinisk kombinationsterapi og ogs\u00e5 v\u00e6re den fremtidige forskningsretning for vores forskningsgruppe.<br \/>\nP\u00e5 baggrund af forel\u00f8big eksperimentel forskning belyser denne artikel yderligere den antiinflammatoriske mekanisme af baicalin mod kolorektal cancer in vitro og in vivo ved hj\u00e6lp af moderne farmakologiske metoder. I betragtning af kompleksiteten af ondartet tumordannelse og forskellene mellem forskellige stammer af tumorceller skal sammenh\u00e6ngen mellem de antiinflammatoriske virkninger af baicalin p\u00e5 kolorektal cancer in vitro og in vivo dog stadig unders\u00f8ges yderligere i flere celle- eller dyremodeller. P\u00e5 samme tid, selvom denne unders\u00f8gelse bekr\u00e6ftede den antiinflammatoriske virkning af baicalin p\u00e5 kolorektal cancer ved at h\u00e6mme Hedgehog-signalvejen og reducere udtrykket af inflammatoriske faktorer, er forskningsindholdet i de to relativt isoleret, og in vitro- og in vivo-effekterne har visse forskelle.
