Og til sidst udvikle sig til nyresygdom i slutstadiet. DKD har en skjult begyndelse, hurtig progression, kompleks mekanisme og d\u00e5rlig prognose. Vestlig medicinsk behandling fokuserer hovedsageligt p\u00e5 at kontrollere blodsukker, blodtryk og blodlipider, men udviklingen af DKD g\u00e5r ikke langsommere, hvilket p\u00e5virker patienternes livskvalitet alvorligt. Da forekomsten af DKD stiger \u00e5r for \u00e5r i Kina, har DKD desuden medf\u00f8rt enorme \u00f8konomiske og sociale byrder, og der er et presserende behov for at udvikle effektive l\u00e6gemidler til behandling af DKD.<br \/>\nDe aktive ingredienser i traditionel kinesisk medicin har betydelige fordele i forhold til at forsinke udviklingen af DKD. Epimediumglykosid kan lindre inflammation i nyrev\u00e6v og forsinke udviklingen af diabetisk nyresygdom (DKD) ved at h\u00e6mme den nukleare faktor kappaB (NF - \u03ba B). Silymarin kan aktivere proteinkinase B (Akt) for at forbedre DKD. Hypericin kan forbedre DKD hos mus ved at fremme polarisering af makrofager fra M1 til M2-f\u00e6notype og differentiering af CD4+T-celler til helper T-celle 2 (Th2) og regulatoriske T-celle (Treg)-populationer. Den bioaktive komponent i lakrids, lakridsflavonoider, kan forbedre diabetisk nyresygdom (DKD) ved at h\u00e6mme jernd\u00f8d og den vaskul\u00e6re endoteliale v\u00e6kstfaktor (VEGF)\/Akt\/ekstracellul\u00e6r signalreguleret kinase (ERK)-vej. Paeoniflorin kan h\u00e6mme forskellige inflammatoriske faktorer s\u00e5som interleukin-1 beta (IL-1 \u03b2), nedregulere proteinudtrykket af fosforyleret Janus-kinase 2 (p-JAK2) og fosforyleret signaltransducer og aktivator af transkription 3 (p-STAT3) i nyreceller, lindre nyrev\u00e6vsinflammation og autofagi-respons og beskytte nyrerne.<br \/>\nCrocin (CRO) er den vigtigste aktive ingrediens i den traditionelle kinesiske medicin safran, som har forskellige virkninger som f.eks. antiinflammatorisk og antioxidant. Unders\u00f8gelser har vist, at CRO kan lindre DKD ved at h\u00e6mme oxidativt stress og inflammatorisk respons. Der er ogs\u00e5 unders\u00f8gelser, der har vist, at CRO kan h\u00e6mme aktiveringen af NLRP3-stien i kardiomyocytter. Det er dog uklart, om det kan behandle DKD ved at h\u00e6mme aktiveringen af NOD like receptor thermal protein domain associated protein 3 (NLRP3) og undertrykke epithelial mesenchymal transition (EMT). Det er en vigtig \u00e5rsag til nyresygdom i slutstadiet. Den stigende forekomst \u00e5r for \u00e5r er blevet en alvorlig medicinsk byrde. Brug af dyremodeller til at studere DKD bidrager til udviklingen af nye l\u00e6gemidler og forbedrer dermed patienternes livskvalitet og reducerer den sociale byrde. Kombinationen af HFD-di\u00e6t og STZ-injektionsinduktion er en klassisk metode til modellering af DKD, og efter vellykket modellering ligner symptomerne dem hos DKD-patienter. Princippet er hovedsageligt relateret til induktion af insulinresistens hos mus ved HFD-di\u00e6t og induktion af betacellenekrose i bugspytkirtlen og nyreskader ved STZ-injektion. 24-timers UTP, Cr og BUN bruges almindeligvis til at evaluere glomerul\u00e6r filtreringsfunktion, og forh\u00f8jede niveauer indikerer nedsat nyrefunktion. I denne unders\u00f8gelse viste DKD-mus alvorlige nyrel\u00e6sioner, herunder \u00f8gede glomeruli, inflammatorisk infiltration af renalt tubulointerstitielt v\u00e6v ledsaget af fibrose, hvilket indikerer vellykket modellering. Efter CRO-intervention blev ovenn\u00e6vnte symptomer forbedret, hvor h\u00f8jdosis CRO viste den mest markante forbedringseffekt, og forbedringsgraden svarede til den for det positive l\u00e6gemiddel MCC950. Andre unders\u00f8gelser har vist, at CRO-intervention i en dosis p\u00e5 25 mg\/kg effektivt kan forbedre bleomycininduceret lungefibrose.<br \/>\nEMT er den direkte \u00e5rsag til nyrefibrose og proteinuri og spiller en vigtig rolle i udviklingen af DKD. Unders\u00f8gelser har vist, at CRO kan h\u00e6mme EMT i leveren og dermed forbedre leverfibrose. Derfor unders\u00f8ger denne unders\u00f8gelse yderligere, om CRO kan forbedre EMT i nyrerne. I denne unders\u00f8gelse blev udtrykket af E-cad i nyrerne hos DKD-mus nedreguleret, mens udtrykket af VIM, \u03b1-SMA og TGF-\u03b21 blev opreguleret, hvilket tyder p\u00e5 en forv\u00e6rring af EMT i nyrerne hos DKD-mus. E-cad er den vigtigste mark\u00f8r for epitelceller, som spiller en rolle i opretholdelsen af epitelcellepolaritet og intercellul\u00e6r adh\u00e6sion. Interstitielle celler udtrykker hovedsageligt VIM, \u03b1 - SMA og TGF - \u03b2 1, hvoraf TGF - \u03b2 1 har en st\u00e6rk pro-fibrotisk effekt. Talrige unders\u00f8gelser har bekr\u00e6ftet, at TGF - \u03b2 1 er st\u00e6rkt udtrykt i DKD-nyrev\u00e6v. Under EMT-processen f\u00f8rer faldet eller forsvinden af epitelcelleadh\u00e6sionsmolekyler og cytoskeletale komponenter til en f\u00e6notypisk transformation af epitelceller, der f\u00e5r mark\u00f8rer for stromaceller. Under forskellige skadestimuli gennemg\u00e5r differentierede epitelceller f\u00e6notypisk transformation for at danne et stort antal fibroblaster og myofibroblaster, hvilket fremmer udviklingen af nyrefibrose. Resultaterne af denne unders\u00f8gelse viste, at CRO kan opregulere udtrykket af E-cad, nedregulere udtrykket af VIM, \u03b1 - SMA og TGF - \u03b2 1 og spille en god h\u00e6mmende rolle i udviklingen af renal EM. Derudover har CRO vist sig at h\u00e6mme EMT i tumorceller gennem MAPK\/ERK-vejen. MAPK\/ERK er ogs\u00e5 en vigtig faktor, der f\u00f8rer til nyrefibrose ved diabetisk nyresygdom. I fremtiden kan der foretages yderligere unders\u00f8gelser af den h\u00e6mmende effekt af CRO p\u00e5 MAPK\/ERK-induceret nyrefibrose for at finde nye m\u00e5l for CRO til at h\u00e6mme EMT.<br \/>\nL\u00e6sionerne af EMT er dynamiske og reversible, og unders\u00f8gelser har vist, at NLRP3-stien er involveret i reguleringen af dens f\u00e6notypeovergang. I denne unders\u00f8gelse blev NLRP3-inflammasomet i h\u00f8j grad udtrykt i nyrerne hos DKD-mus, hvilket forv\u00e6rrede den inflammatoriske reaktion i nyrerne hos DKD-mus. NLRP3-inflammasomet er et multiproteinkompleks, der best\u00e5r af NLRP3, ASC og caspase-1. Aktiveret NLRP3-inflammasom spiller en vigtig rolle opstr\u00f8ms for DKD. N\u00e5r NLRP3 aktiveres, fremmer det samlingen af NLRP3-inflammasomkomponenter (NLRP3, ASC, caspase-1) og aktiverer derved pro-caspase-1 til at danne aktiv renset caspase-1, som spalter og modner de inflammatoriske faktorer IL-1 \u03b2 og IL-18. Modnet-IL-1 \u03b2 kan stimulere syntesen af TGF - \u03b2 1, som er en n\u00f8glefaktor i udviklingen af EMT. TGF - \u03b2 1 kan fremme syntesen af forskellige proteinkomponenter i den ekstracellul\u00e6re matrix og forhindre deres nedbrydning og derved forv\u00e6rre processen med renal EMT og fremskynde dannelsen af renal fibrose. Desuden har unders\u00f8gelser vist, at udtrykket af \u03b1-SMA i NLRP3-knockout-mus er markant nedreguleret. Overekspression af NLRP3 h\u00e6mmer ekspressionen af E-cad og fremmer ekspressionen af \u03b1-SMA, mens VIM har vist sig at fremme aktiveringen af NLRP3. Alle disse antyder, at der er en gensidig reguleringseffekt mellem NLRP3 og EMT, og at h\u00e6mning af NLRP3 kan lindre renal EMT betydeligt. Og denne unders\u00f8gelse viste, at CRO-intervention effektivt h\u00e6mmer udtrykket af NLRP3-inflammasomrelaterede faktorer og ud\u00f8ver gode antiinflammatoriske virkninger, samtidig med at der ud\u00f8ves anti-EMT-effekter. Aktiveringen af NLRP3 kr\u00e6ver inddragelse af dobbelte signaler. For det f\u00f8rste aktiveres transkriptionsfaktoren NF - \u03ba B og kommer ind i kernen, hvilket fremmer udtrykket af gener som NLRP3 og IL-1 \u03b2 (f\u00f8rste signal); bagefter, under stimulering af faktorer som K + og ROS, initieres NLRP3-inflammasomsamling (andet signal). Interessant nok har tidligere unders\u00f8gelser vist den h\u00e6mmende effekt af CRO p\u00e5 NF - \u03ba B. S\u00e5 om CRO kan undertrykke NLRP3-induceret EMT og forbedre DKD ved at h\u00e6mme NF - \u03ba B (det f\u00f8rste signal) kan blive en fremtidig forskningsretning. CRO kan h\u00e6mme NLRP3-aktivering og forbedre peritonitis ved at reducere produktionen af mitokondrielle reaktive oxygenarter. I mellemtiden er CRO's antioxidante og mitokondrielle reparationsfunktioner blevet rapporteret i forskellige modeller. S\u00e5 der er behov for yderligere forskning for at bekr\u00e6fte, om CRO kan forbedre EMT i DKD-modellen ved at reducere mitokondriel ROS-produktion og derved blokere aktiveringen af det andet signal fra NLRP3 og h\u00e6mme dets aktivering.
